Fall Registration Checklist 2010-2011 Seasonal Year

 

Please read below for registration information for your team.  There are four different registration scenarios as follows:

 

A:  Teams playing Cal Soccer league (CYSA Bay or Coast) ONLY

B:  Teams playing Norcal fall league (US Club) ONLY

C:  Teams playing BOTH Cal Soccer and Norcal fall leagues

D:  Teams playing BOTH Cal Soccer fall league and participating in US Club sanctioned events, including Norcal State Cup

 

Determine which of the above schedules your team plans on participating in the fall and follow instructions below for that scenario. If none of the above scenarios matches your team’s schedule or if you have any questions regarding registration or fees, please contact Anne-Marie Moore at scheduler@cvysl.org.

 

A.  Teams playing Cal Soccer (CYSA) league ONLY in Fall 2010

 

Please submit the following to Angie Onate:

 

  • Two copies of wet signed (in color ink) 1601 player registration form for each player.  Here is the link:

  • Copy of birth certificate or passport for each player regardless of whether or not they have played on this team in the past.  No other forms of proof of age will be accepted.  If your team has any foreign born players, please make sure to include Form 1627-Foreign Document Translation Form for each of those players.  Here is the link:

  • One copy of wet signed 1628 Adult Disclosure Form for each adult that will have a passcard.  Here is the link:

  • Photo of each player and adult to be carded.  Photos should be cut to size, 1 inch by 1 inch, head shot only, no sunglasses, hats or shadows.  The names should be written on the back of each photo and the photos should all be placed in one plastic baggie.

 

Birth certificates should be placed in alphabetical order and paper clipped together.  1601s should be placed in alphabetical order and paper clipped together.

 

Include a copy of the most recent coaching license for the head coach and for each assistant coach.  Remember that the head coach for U16 teams and older, regardless of playing division, is required to have a D license.

 

Cost for the fall Cal Soccer season is $150 per player.  Teams should submit one check to Angie for the entire team ($150 x number of players) made payable to CVYSL. 

 

All of the above should be submitted to Angie in a large envelope with the manager’s name, team name, email, and telephone number.  Please allow a minimum of 3 weeks for processing.

 

If your team name has changed since last fall, please make sure to let Angie know both old and new team names.  If your team is part of a tiered program, please make sure to include the color (red, white, or blue) as well as the playing year (98, 99 etc).

 

B.  Teams playing Norcal league ONLY in Fall 2010

 

The cost for the fall Norcal season is $140 per player.  Teams should submit one check to Leslie Duquette for the entire team ($140 x number of players) made payable to CVYSL.

 

In addition, teams must submit $15 for each coach/assistant coach/manager/staff to be carded through US Club.  Each adult must fill out the on-line risk management questionnaire (background check) on the US Club website before a passcard can be issued.

 

Once Leslie receives your team list and check, she will request passcards in .pdf format from US Club.

 

Teams are responsible for uploading player information and birth certificates as required to the Norcal/US club site and paying the $150 on-line team registration fee to play Norcal fall league.  This fall, referees for Norcal fall league games will be paid by Central Valley.

 

C.   Teams playing in BOTH Cal Soccer and Norcal leagues in Fall 2010 

 

Submit all fees and paperwork as indicated in Part A above for the Cal Soccer league to Angie Onate.  Please allow a minimum of 3 weeks to process CYSA registration paperwork.

 

Upload all player information and birth certificates as required to the Norcal/US Club site.  All coaches/assistant coaches/managers/staff affiliated with the team that need passcards are required to fill out the on-line risk management questionnaire (background check) on the US Club website.

Submit additional payment to Leslie Duquette in one check made out to CVYSL as follows:

 

  • U9 through u11 teams:  $400 plus $12 per player to be carded through US Club
  • U12 and older teams:  $800 plus $16 per player to be carded through US Club
  • All age groups:  $15 for each adult to be carded through US Club

 

The additional $400 for u9 through u11 teams covers referee costs for 5 home Norcal league home games; the additional $800 for u12 through u18 teams covers referee costs and game field fees for 5 home Norcal league games.  

 

Once Leslie receives the additional payment as noted above and your team list, she will request passcards in .pdf format from US Club.  Please allow a minimum of 3 weeks to process passcards for Norcal/US Club.

 

D.  Teams playing in Cal Soccer league in Fall 2010 and any sanctioned US Club events, including Norcal State Cup for Fall 2010

 

Submit all fees and paperwork as indicated in Part A above for the Cal Soccer League to Angie Onate.  Please allow a minimum of 3 weeks to process CYSA registration paperwork.

 

Upload all player information and birth certificates as required to the Norcal/US Club site.  All coaches/assistant coaches/managers/staff affiliated with the team that need passcards are required to fill out the on-line risk management questionnaire (background check) on the US Club website.

 

Submit additional payment to Leslie Duquette in one check made out to CVYSL for the following:

 

For u9 through u11 teams, $12 per player to be carded through US Club.

For u12 through u19 teams, $16 per player to be carded through US Club.

All age groups:  $15 for each adult to be carded through US Club.

 

Once Leslie receives the additional payment as noted above and your team list, she will request passcards in .pdf format from US Club.  Please allow a minimum of 3 weeks to process US Club passcards.

 

Norcal State Cup participants will be responsible for paying referees, in cash at the field, for their Norcal State Cup home games. 

 


Additional information for Cal Soccer CYSA teams

 

Please allow a minimum of 3 weeks for processing of CYSA (Cal Soccer) paperwork, assuming that all of the submitted paperwork is in order as described in Part A above.

 

Because Angie will be out of the office for two weeks in July (July 6 through July 20), please be aware of the following deadlines pertinent to summer CYSA tournaments:

 

  • Teams playing in a CYSA event on July31/August 1, all paperwork and fees need to be submitted to Angie by June 25 so that the goldenrod and passes will be ready by July 30 tournament check-in.

 

  • Teams playing in a CYSA event on August 7/8 weekend, all paperwork and fees need to be submitted to Angie by July 2 so that the goldenrod and passes will be ready by August 6 tournament check-in.

 

  • Teams playing in a CYSA event on August 14 or later should have paperwork in to Angie at least 3 weeks prior to your first CYSA event, including fall Cal Soccer League.

 

Please remember that Angie processes league paperwork at her place of business and as such, CVYSL team representatives are expected to be respectful to Angie and her co-workers at all times.  Please have your paperwork in order before entering Angie’s office.  Angie can be reached via email at compreg@cvysl.org.  Checks and paperwork should be delivered or mailed to:

Additional information for teams playing Norcal/US Club

Please note that coach/assistant coach/manager/staff passcards are good for two seasonal years and that a carded adult needs only one passcard to be affiliated with any team in the club (Central Santa Clara Valley Soccer Club).  Therefore, if your team is part of a tiered program or your coach/trainer/manager works with another team within CSCVSC, then you do not need to obtain duplicate passcards for that individual.
